Interactive PDF Fields
There are five categories of interactive fields used in Interactive PDFs: Text fields, columns and tables, drop-down lists, radio buttons, and check boxes.
Text Fields
- Any underline should be made into a text field unless otherwise indicated.
- Occasionally, the book design has not used underlines to indicate a space for answers, but left space blank to be written in instead. These should still be converted to text fields and are noted in the individual title instructions.
- Anything headed by “Notes” with lines that go across the entire page should be captured as
one text field with multiple lines, e.g.,
Columns and Tables
- When the form is designed as a column - or table-based worksheet each column
or cell should include at least one text field. If the column is divided into sub-categories,
each one should have a text field, e.g.,
Drop-down Lists
- Options that are limited, e.g., “Y N” or “(0-10)” should be captured as drop-down lists with those options, e.g., option 1: Y, option 2: N.
Radio Buttons

Scales: There should be radio buttons inserted before each selectable option, e.g., a
numbered scale that asks a reader to rate his feelings from between 1-7, should look like this
in the print:
