In many cases in the Pocket Notes and Cards series, the chapter divisions are not readily apparent. The main titles in the Table of Contents will correspond to the chapter titles. These chapters do not always occur on a new page and they do not always correspond word for word. Capture main divisions (sometimes enumerated and with page numbers as shown above) as chapter element.
In the sample text below Risk Assessment and the NCEP/ATP-III Nine-Step Approach to Dyslipidemia corresponds to the title of chapter 4.
