Skipping levels of hierarchy
For complex hierarchies where heading levels are skipped, use dummy
div[1-7,N] elements to represent each skipped heading level.
A dummy
div[1-7,N] element contains no content of its own (e.g. no
titleGroup element), just the next level div[1-7,N]
element.
