Capture the title of the document in the metadata and not in the body of the document.

This instruction extends the OCW general instruction.
If the title in the commentary document includes a reference to an editorial footnote, capture these notes as a comment element within regMeta, exclude them from the document text, and renumber other editorial footnotes appropriately.
