Page breaks within tables
For tables which flow across multiple pages capture page breaks in the first text node on each page.
Capture page break as a combination of page number processing instructions and page milestones in the p element of the first entry on a page.
If the content of a table cell looks to span across to a new page, create a new row with the page number processing instruction at the start of the first cell on the new row.

Sometimes in print if a table spans multiple pages its heading is repeated immediately after the page break. Do not capture repeated headings in the XML.
