Book chapter
Chapter references must have class='bookChapter'.
Chapter references are distinguished from book references by including the chapter title, and often a chapter author different from the author/editor of the overall book. There may also be a page reference.
The order in which they appear in the reference, and the way they are punctuated, is variable. See the examples.
