Capture main titles with the xrefGrp xref elements.
Only the main titles (e.g. chapters with page numbers) need to be captured with the xrefGrp xref elements. All other titles can be left as is. In the example below, NCEP/ATPIII Nine-Step Approach to Dyslipidemia and Statins -- Mechinism of Action are considered main titles.
