Forms in appendices (Medical Libraries)

In Medical Libraries, some books contain forms or surveys at the end of the book in an appendix. Capture these forms as images; one figureGroup per form.

If the form spreads over several pages then capture a separate image file for each page. For each image use a separate graphic element in the figureGroup.
