div1 division headings include running heads (OHPDT)

Unless the running head is the same as the div1 division title, prefix the title with the running head followed by a colon and a space : .

Prefix the title of each div1 with the running head for the page such that the div1 division title is in the format:

running head: div1 head

Where the running head and the div1 division heading are the same, only capture the division heading.

Capture the BNF reference that appears on the right hand side of the running head as a subtitle of the div1.
