URLs in headnote references

Capture URLs added to a case report headnote references in a url attribute of the bibItem element. Capture other URLs in a url child of bibItem.

Where a references in a headnote field includes a URL, it appears in square brackets and in bold face, either after the end of the reference text, or after the bracketed link type information.

Capture the content of the brackets as the value of the url attribute of the bibItem. Do not capture the URL in the text.

If there is more than one URL, capture them all in the same url attribute, separated by vertical bars |.
