Do not use the bold-italic bi element in OSO titles.
In OSO titles, only use extRef to refer to another OSO title. The format of the docRef and ref attributes follow the same pattern as id attributes in the source document (but with a different ISBN).
Tables in OSO titles normally have horizontal rules at the top and bottom. In XML this is coded using the attribute frame="topbot".
