div1 headings only appear in Chapter 2 and Chapter 4. In Chapter 2 these are The Pre-Travel Consultation, Self-Treatable Conditions, and Counseling & Advice for Travelers. In Chapter these are the names of the continents (i.e. Africa, Asia, etc).

Use dummy division levels to account for the missing div1 headings in the other chapters, as described in Skipping levels of hierarchy. All other chapters begin at the div2 level.

div2 elements have authorship. Capture this in creatorInfo on titleGroup.

There is some content headed by Perspectives or For the Record. These should be captured using boxedMatter and should also be captured as features. Assign role="FT1012" to Perspectives boxedMatter elements. Assign role="FT1013" to For the Record boxedMatter elements. Perspectives and For the Record must be captured as the supertitle.
